Donor Recommendation Form

Making a grant from your donor advised fund is very easy.  Simply fill out the donor recommendation form below and either mail or fax it to our office.  Once we receive your form, our office will mail the grant along with a letter to your charity of choice.

Grants are typically made within one week and will be distributed from our host, the Greater Salina Community Foundation.  If you need it processed sooner than that, please call our office, 785-445-3611

Donors can also make grant requests online through their Donor Portal account.  Don’t have one set up?  Contact the Foundation and we will set up one for you.

Community foundation accepting fall grant applications HAYS, Kan.—The Heartland Community Foundation, which serves Ellis, Rooks and Trego Counties, is accepting applications for fall grants. Online applications open September 15 and are due Sunday, November 15, 2020. Last fall, the foundation awarded Redbud Village funding to update their dining facilities.
